Epidemiological Alert: Emergence and increase of new combinations of carbapenemases in Enterobacterales in Latin America and the Caribbean (22 October 2021)

Resumen
Given the change in the geographic distribution of carbapenemases and the emergence and dissemination of bacteria that produce more than one of these enzymes, the Pan American Health Organization / World Health Organization (PAHO / WHO) emphasizes the importance of appropriate microbiological diagnosis and the effective and articulated implementation of infection prevention and control programs, as well as regulations for the optimal use of antimicrobials. During the COVID-19 pandemic, the emergence of extensively antimicrobial-resistant microorganisms and an increase in the incidence of resistance to carbapenems, possibly related to the increased use of broad-spectrum antibiotics in patients with COVID-19, has been documented. At the same time, an increase in the rate of device-associated healthcare-associated infections has been observed in intensive care units (ICUs), mainly due to central vascular catheter and mechanical ventilation. A situation summary of the situation in Latin America and the Caribbean is provided here. Ante el cambio de la distribución geográfica de las carbapenemasas y la emergencia y diseminación de bacterias productoras de más de una de estas enzimas, la Organización Panamericana de la Salud/Organización Mundial de la Salud (OPS/OMS) enfatiza la importancia del diagnóstico microbiológico apropiado y la implementación efectiva y articulada de programas de prevención y control de infecciones, así como de regulaciones para la optimización del uso de antimicrobianos. Durante la pandemia de COVID-19 se ha documentado la emergencia de microorganismos extremadamente resistentes y un aumento de la incidencia de resistencia a carbapenémicos, posiblemente relacionados con el incremento del uso de antibióticos de amplio espectro en pacientes con COVID-19. Al mismo tiempo, se ha observado un aumento en la tasa de infecciones asociadas a dispositivos en las unidades de cuidados intensivos, principalmente por catéter vascular central y ventilación mecánica. Aquí se presenta un resumen de la situación en Latinoamérica y el Caribe.
