Abstract
La Región ha hecho grandes progresos en la expansión del tratamiento ARV y se encuentra cerca del cierre de las brechas de cobertura universal al tratamiento. Queda, por plantearse si la región está aprovechando al máximo los beneficios que ofrece el tratamiento. En concreto, en materia de reducción de nuevas infecciones y de la mortalidad por VIH. Para que ALC pueda llegar al fin de la epidemia de VIH debe abordar, entre otros, los retos implícitos en la expansión del tratamiento. El primer reto es lograr un acceso más temprano al diagnóstico del VIH. Para ello se deben incrementar los esfuerzos para ampliar la prueba y consejería del VIH. El segundo reto es conseguir una atención de calidad que maximice la retención en TAR y la reducción de la carga viral a niveles indetectables para todos los pacientes. El acercamiento de los servicios a la población, la participación comunitaria, el monitoreo virológico son elementos clave para el logro de esta atención de calidad.
 
The Region has made major advances in expanding ARV treatment and has nearly breached the gaps in universal coverage. The question remains as to whether the region is taking full advantage of the benefits of treatment, specifically with regard to reducing new HIV infections and mortality. To end the HIV epidemic, LAC should address, among others, the implicit challenges of expanding treatment. The first challenge is to achieve earlier access to HIV diagnosis. To this end, efforts to expand HIV testing and counseling should be increased and appropriate strategies to reach the different populations should be employed. The second challenge is to achieve quality care that maximizes retention in ART and reduces the viral load to undetectable levels in all the patients. Getting the services to the people, community participation, and virological monitoring are key elements for achieving quality care.
